    Eligibility: The candidate should have passed 10+2 examination of a recognized Indian Univ. / board with at least 50% marks in English and 50% marks in Physics, Chemistry and Biology taken together. The candidate should also have qualified UPMT-2005 or other state government PMT/CPMT 2005 with a minimum of 50% marks. For CBSE-AIPMT-2005 should have qualified in merit/waitlist.
    How to apply: The candidates shall APPLY SEPARATELY FOR EACH CATEGORY in the prescribed form (which can be downloaded from the website) along with an application fee of Rs. 4,000/- (Four thousand Only) for CATEGORY A and Rs. 15,000/-(Fifteen Thousand only) for CATEGORY B payable in Cash/A/c payee Draft in favour of Secretary, Uttaranchal Forest Hospital Trust at Haldwani latest by 05.09.2005 (17.00 hrs.) by hand/courier/ post along with the application fee enclosing the following documents (1) 10th pass certificate (2) 12th pass (Intermediate) certificate and marksheet (3) PMT/CPMT/AIPMT-2005 admit card and mark sheet (4) Two recent photograph. If a candidate wants to apply for both the Categories, he/she shall have to apply separately in two applications for each category along with separate application fee of Rs. 4,000/- (Rs. Four Thousand only) and Rs. 15,000/- (Rs. Fifteen Thousand Only) for Category A and Category B respectively. Applications along with requisite fee and documents mentioned above must reach the Office of Secretary, Uttaranchal Forest Hospital Trust, Rampur Road, Haldwani (Nainital), Uttaranchal, India, Pin- 263139 on or before 17.00 hrs. of 05.09.2005. Applications received after 17.00 hrs. of 05.09.2005 shall not be entertained. Applications received through Fax/E-mail will not be accepted.     Comments: Selection Procedure: -Merit list separately for category (A) and (B) shall be prepared from amongst the qualified applications received on or before 17.00 hrs. 05.09.2005 on the basis of percentage marks obtained in the concerned PMT.
